Given primary site name: primary.net
Primary site has add-on domain named: addon.com
I have Frontpage extensions installed on primary.net and addon.com
I have a form on primary.net that emails results and it works just fine!
I copied the form to addon.com but it does not work
I re-installed FPSE on addon.com... now I don't even get an error... just a blank pageFrontPage Error.

I am pretty sure it has something to do with the server side FPSE setup. When the form does not work you can see www.addon.com/_vti_bin/shtml.exe/form.htm in the address bar of the browser (Firefox). In IE you get a Page not found error.
I believe there is something wrong with the way FPSE are being installed to add-on domains or something as simple as the setupfp5 script not merging correctly - here is the log from re-installing FPSE:
If I use the url primary.net/addon/form.htm it will work the same way that the primary.net/form.htm works...Saving .htaccess file: /home/primary/public_html/addon/.htaccess

Here are the steps I followed to resolve this issue:
1. Delete/Rename the .htaccess file at the root of the add-on domain, i.e., www.addon.com/.htaccess
2. Clean-out old FP files/directories
3. Install FPSE to add-on domain.
4. Open add-on domain/web in FP
5. Recalculate Links
This worked for me... hope it can help anyone else with this issue...
