Given primary site name: primary.net
Primary site has add-on domain named: addon.com

I have Frontpage extensions installed on primary.net and addon.com
I have a form the emails results to owner@primary.net and it works just fine!
I copied the form to addon.com and have the results set to email owner@addon.com but it does not work

I have talked to HM Support a number of times without success.
Originally I got an error message after clicking the Submit button:

FrontPage Error.
User: please report details to this site's webmaster.
Webmaster: please see the server's system log for more details.
The last support tech said I should re-install FP, which I did... now I don't even get an error... just a blank page

I have asked that a system admin just look at the server system log to no avail...

If I use the url primary.net/addon/form.htm it will work the same way that the primary.net/form.htm works...

I am pretty sure it has something to do with the server side FP setup. When the form does not work you can see http://www.addon.com/_vti_bin/shtml.exe/form.htm
in the address bar of the browser (Firefox). In IE you get a Page not found error.

I believe there is something wrong with the way HM is installing FP to add-on domains or something as simple as the setupfp5 script not merging correctly - here is the log from re-installing FP:

I am trying to get away from FP but time... ... not enough time...
Does anyone have a add-on domain, using FP forms working on HM? how about other sites??
